Your name, phone number, treatment interest, preferred time or day, communication language and consent to follow up. It does not request medical records, images or payment details.
How the details are used
Appointment requests are stored in the website administration area and accessed by authorised site administrators to arrange contact. If email alerts are enabled, the clinic receives a notification of a new request without the personal request details in the notification.
Keeping an appointment request
Requests are scheduled for automatic deletion after 30 days. Deletion depends on the hosting environment running scheduled tasks. This form is not a clinical record; information collected during care is handled through the clinic’s separate processes.
Cookies and external platforms
WordPress administration uses login cookies. The form uses technical controls to limit repeated submissions. This theme includes no advertising trackers. Playing a video, loading a map or opening WhatsApp also engages the external platform’s privacy policy. Embedded videos and maps are not loaded before you choose to open them.
Privacy requests
Use the contact page to ask questions or request correction or deletion of appointment details. The clinic verifies the requester’s identity before acting. This page should be updated when the collection process or connected services change.
